Take the SIM card out, and reposition it until it enters in the tray properly. Push the tray all the way back to make sure that the SIM card is fully within proximity of the connectors.

In case you are sending an MMS or SMS to several people at once, you should also make sure that ALL of them have their area codes also inserted with the number. If one of them is recorded with faulty information, automatically the iPhone will be unable to send the SMS/ MMS.

Another issue usually appears when you port your number form one carrier to another. in case of the said issues you need to contact your current carrier and make sure that the porting process has been completed without mishaps.
